Начала свое обучение в Школе американского балета в возрасте восьми лет. Позже занималась в Школе балета Жаклин Кеннеди при American Ballet Theatre. Сара была солисткой балета Дрезденской государственной оперы.

Дебютом Сары Хэй в кино стала роль в знаменитом фильме-драме режиссёра Даррена Аронофски «Чёрный лебедь» (2010). В 2015 году актриса была приглашена на одну из главных ролей в сериале о жестоком мире профессионального балета «Плоть и кости 1 сезон» (Flesh and Bone) от сценариста и исполнительного продюсера рейтингового проекта «Во все тяжкие» Мойры Уолли.
